Dispensing Optician License

Initial Application for a License: If you are a licensed Nevada apprentice and have completed all your apprenticeship requirements, please log in to your account to access the Dispensing Optician License Application under the "Forms" tab.

Inactivation/Reactivation of a License:

  • To inactivate your license, log in to your account and go to the Forms tab to submit your request. There is a $300 fee to inactivate a license for a five-year period. At the end of the initial 5-year term, the license will expire if it is not either reactivated or renewed for a second 5-year inactive term. A license may remain inactive a total of 10 consecutive years before it will expire permanently. An inactive license does not need to be renewed annually, however, it is illegal to dispense prescription eyewear with an inactive license.
  • To reactivate your license, log in to your account and to go the Forms tab to complete the current year’s renewal form. You will need to submit the renewal fee and any required CE credits.
  • Please note: A Limited License as a Dispensing Optician may not be inactivated or reactivated.

Reinstatement of a License: An expired Dispensing Optician License may be reinstated within one year of its expiration upon payment of a delinquency fee. To reinstate an expired license, log in to your account and go to the Forms tab to complete the current year’s renewal form.

Transition of a Limited License to a Dual License: If you are a Nevada Optician with a Limited License (for dispensing spectacles only) and want to obtain a dual license to dispense both spectacles and contact lenses, you must obtain the required contact lens education and training and pass the NCLE Basic and Practical certifying exams, as well as the current Nevada Law and Ethics Exam. Once you have completed your licensing requirements, submit this form to the Board office along with all required documentation.
